We’re excited to have you join the TSX team for the upcoming season! This form helps us collect important information needed for trip planning, safety, emergency preparedness, insurance, and required reporting, including:

  • Contact and emergency contact information
  • Medical information that may be relevant for trek planning or in the event of an emergency
  • Documents and certifications required for compliance and reporting (e.g., driver’s license, WFR, Food Handler Certification)

Please complete the form to the best of your ability. If you do not yet have certain documents available, you may submit the form with the information you have and return later to upload additional items as they become available.

The information collected in this form is used solely for operational, safety, compliance, and insurance purposes. Access is limited to TSX leadership and designated staff and is reviewed only as needed to support trip operations and emergency response. Information is retained only as long as necessary and is handled in accordance with applicable privacy laws.
